Permission must be sought prior to photographing any persons under 18. All photos and video of persons under 18 used in Lifelites publications, either online or in print, must be accompanied by a signed consent form.

Parent, carer or responsible adult consent

• I confirm that I have been made aware of Lifelites’ photography and filming policy, and Privacy Policy, which are available to view at www.lifelites.org.
• I understand and agree that pictures or videos of my child will be used, without compensation, in printed or electronic materials related to the role and function of Lifelites, including fundraising, campaigns and appeals.
• I understand that they will not be used by a third party outside of the function of Lifelites without my explicit written permission.
• I understand these images or videos will be stored by Lifelites and authorised third parties strictly for the purpose for which they were collected.
• I understand I can withdraw consent for my child’s image to be used or shared without explanation at any time.
• I understand that if I withdraw consent for my child’s image to be used or shared in the future, it may not be possible to remove images that have already been published or distributed.
• I understand that at many events, Lifelites’ and their official representatives will reasonably wish to take wide angle, general photos during, or at specific points in the event
• I agree to my child’s photograph being used within Lifelites’ other printed publications.
• I agree to my child’s photograph being used on Lifelites’ public website.
• I agree to my child being videoed for use on Lifelites’ public website
• I agree to my child’s photograph being used on Lifelites’ public social media pages
• I agree to my child being videoed for use on Lifelites’ public social media pages.
• I understand the potential risks associated with the use and distribution of these images.
